Reasons and Methods for Removing Buried Sutures

Common reasons for wanting to remove buried sutures include dissatisfaction with the shape or functional side effects, such as a pulling sensation caused by the threads.

The removal process involves making 1 to 3 tiny micro-holes in the skin to locate and extract the sutures.

Buried Suture Removal Case 1

This is the appearance before the removal procedure. In this case, the sutures were removed through a single micro-hole.

The patient had undergone buried suture double eyelid surgery 3 years and 6 months ago but wanted to undo the folds because they felt their eyes looked ‘greasy’ or overly heavy.

Since it had been a long time since the initial surgery, there was some uncertainty about whether the folds would fully release, but we proceeded with the removal as the threads were still extractable.

This is the appearance immediately after the removal. Right after the procedure, the eyelids start to look slightly loosened, but the folds do not disappear completely at once.

However, significant loosening can be seen after just a few days.

This is the appearance 3 days after the procedure. The folds will continue to loosen over time, usually becoming significantly undone within a week to a month.

In very rare cases, the folds may persist even after the sutures have been removed.

Buried Suture Removal Case 2

This patient had surgery 11 months prior. They were unhappy with the shape and wanted to return to their original natural double eyelid line.

When we remove the sutures, we take ‘proof shots’ of the extracted threads to reassure the patient that they have been successfully removed. We always show the removed sutures to the patient.

Buried Suture Removal Case 3

This patient originally had thin, inner double eyelids and wanted to undo the surgical folds. I always emphasize to patients that this is a ‘suture removal surgery’ rather than a ‘fold removal surgery.’

While removing the sutures usually leads to the folds undoing, there are cases where they do not, so this is an important point to keep in mind.

In this case, the sutures were transparent. It is generally easier to find sutures that are black, blue, or otherwise colored. Transparent sutures are relatively more difficult to locate.

Buried Suture Removal Case 4

This patient had surgery at another clinic, but the line was too high and they were dissatisfied. They visited us the very next day.

We removed the buried sutures and performed a revision double eyelid surgery on the same day. If it has only been a short time since the initial surgery, early revision is possible.

However, if some time has passed, we recommend removing the sutures first and then deciding whether to proceed with a new buried suture method or an incisional method later.

If the buried thread is removed, will the double eyelid immediately loosen?

Yes, in most cases, removing the thread will cause the double eyelid to loosen. You may feel a slight loosening immediately after the surgery, and it usually loosens significantly after one week to one month. However, it is very rare for the double eyelid not to loosen even after the thread is removed, so please keep that in mind.

Is it possible to remove the buried thread even after more than 3 years since the surgery?

Yes, thread removal is possible even if a long time has passed since the surgery. There are actual cases where buried threads were successfully removed through a microscopic hole even after 3 years and 6 months. However, if a long time has passed, whether the double eyelid will completely loosen may vary from person to person.

How is the buried thread removal surgery performed?

It is performed by making 1 to 3 tiny holes in the skin. Through these small holes, the previously tied threads are located and removed. Since no incision is made, there is less burden of scars and the recovery is relatively fast.

Can I have double eyelid revision surgery immediately after buried thread removal?

Yes, if it's an early stage shortly after the initial surgery, immediate early revision is possible on the same day. However, if a considerable amount of time has passed since the surgery, it is recommended to first remove the threads to stabilize the eyes and then proceed with revision surgery using the buried method or incisional method later.

Can transparent buried threads also be removed?

Yes, they can be removed, but they are relatively harder to find compared to colored threads. Colored threads such as black or blue are easily visible and can be found smoothly, whereas transparent threads are not easy to distinguish, requiring more precise techniques from the medical staff during surgery.
