Today, I would like to talk about male ‘monolid’ ptosis correction.

Through actual patient cases, I will explain how these patients underwent surgery, how the plans were established, and what kind of results were achieved.

First of all, this patient looks very sleepy, doesn’t he?

He looks tired, and because his skin is relatively thick, a visible double eyelid would likely loosen easily if created.

Currently, only about half of the pupils are visible, and there are no double eyelids.

In this case, since the primary goal was to make the eyes look larger, we focused on making the eyes clear and removing thick soft tissue.

While we often talk about ‘removing fat,’ we also removed some hypertrophic orbicularis oculi muscle (the thick tissue around the eye) and ROOF fat, which is typically difficult to remove. This creates a lighter appearance.

The goal was to create a hidden double eyelid that is tucked in as much as possible to keep it invisible while making the eyes look sharp.

This patient is in a similar situation.

The eyelids are very thick, and only half of the pupils are visible.

The goal for this patient was also to keep the double eyelid as invisible as possible while making the eyes look sharp and eliminating the sleepy look to improve the overall impression.

Similarly, we performed monolid ptosis correction to minimize the visibility of the eyelid crease while focusing on removing the thickness of the eyelids.

This is the appearance at 2 months post-op; some swelling still remains.

This patient used his forehead muscles extensively to open his eyes.

When eyes don’t open easily, people tend to compensate by using their forehead. Most people with small eyes do this, and once the eyes are enlarged, the forehead muscles relax and the brows settle lower.

This patient underwent monolid ptosis correction along with epicanthoplasty and lateral canthoplasty.

After surgery, the forehead is used less, and the eyes become clear. Since this is only one week post-op, swelling is still present.

Before surgery, the area right under the eyebrows looked sunken because the forehead was being pulled up. After surgery, as the forehead relaxes and moves down, the area under the eyebrows appears fuller.

While ptosis correction can increase eyelid volume, the sunken look under the brows disappears primarily because the forehead muscles are no longer strained.

This patient had a slight double eyelid on one side and multiple messy folds on the other, making the eyes look loose and slightly sleepy.

He wanted to eliminate these existing wrinkles while keeping the double eyelid as invisible as possible.

While non-incisional methods are used in very rare cases, an incisional approach is usually required to clean up these folds effectively.

This patient’s eyes were already quite clear, but he had very deep and high double eyelids.

In Korea, this can sometimes give off a ‘strong’ or ‘harsh’ impression, so the patient wanted to hide the double eyelids as much as possible.

He didn’t necessarily need his eyes to be sharper, but he wanted the line lowered so that the double eyelid would be hidden.

This patient had previously undergone double eyelid surgery at another clinic.

While the sagging skin was improved, visible double eyelids on men often don’t align with local aesthetic preferences. Therefore, we performed a procedure commonly known as ‘du-jul-tta-gi’ to lower the line so it is covered when the eyes are open.

Technically, ‘du-jul-tta-gi’ is not a formal medical term; it refers to a ‘line-lowering surgery that preserves the scar.’

This method involves creating a new line below the existing one and releasing the old scar tissue to lower the crease.

Frequently Asked Questions

Can thick eyelids also be improved with male non-double eyelid ptosis correction?

Yes, improvement is possible. If the eyelids are thick, to make the eyes clearer, not only unnecessary fat but also hypertrophied orbicularis oculi muscle and loop fat tissue can be removed together to create a lighter-looking eye shape.

I use my forehead to open my eyes due to ptosis. Will there be a change after surgery?

Yes, there will be a change where forehead usage is reduced. When the strength of opening the eyes is enhanced through ptosis correction, the forehead muscles are used less, causing the forehead to lower, and as a result, you can see the effect of the sunken area below the eyebrows becoming plumper.

If there are multiple double eyelid folds, is non-incisional surgery possible?

Although non-incisional surgery is rarely possible, incisional methods are mostly required. To neatly remove multiple folds and hide the double eyelid so it's not visible, it is common to proceed with incisional non-double eyelid ptosis correction.

My existing double eyelid is too deep and high. Can the line be lowered?

Yes, surgery to lower the line is possible. Through a method called 'double line incision' (두줄따기), where a new line is created below the existing double eyelid line and the scar is released, the double eyelid can be corrected to be covered and not visible when the eyes are open.
