First, eyelid retraction correction is a surgical method required when the pupils are excessively exposed due to overcorrection during a previous ptosis correction surgery.

When ptosis overcorrection occurs, it is not simply a matter of ‘undoing’ the previous correction. Instead, it involves dissecting the Müller’s muscle beneath the levator muscle and even the conjunctiva below that, before performing a new, precise ptosis correction.

This is the appearance before surgery.

The left eye was overcorrected, resulting in uneven pupil exposure. The patient wished to correct this asymmetrical, ‘mismatched’ look.

In what cases is eyelid retraction surgery necessary?

It is necessary when blepharoptosis correction is overcorrected, and too much of the pupil is exposed. Simply undoing the existing blepharoptosis correction is not enough; the Muller's muscle under the levator palpebrae superioris, which opens the eye, and the conjunctiva must all be dissected, and then blepharoptosis correction must be performed anew to restore the original natural eye shape.

Does overcorrected blepharoptosis correction return to normal just by untying the knot?

No, simply undoing the blepharoptosis correction does not return it to its original state. To improve the overcorrected condition, eyelid retraction surgery, which meticulously dissects the Muller's muscle under the levator palpebrae superioris (the muscle that opens the eye) and the conjunctiva beneath it, must be performed, followed by new correction.
