Lowering the Eyelid Line: Various Cases and Surgical Methods for Eyelid Revision

Today, we will look into various cases of lowering the double eyelid line, which is often performed when overcorrection occurs after ptosis correction.

We have prepared several cases to discuss today,

and we hope this will be helpful for those who find themselves in similar situations.

Cases where the ‘two-line method’ (skin-fold preservation) is necessary due to insufficient skin

In this patient’s case, the existing double eyelid line was high and appeared wrinkled.

It looks as though the double eyelid has slightly unraveled, and the line is positioned very high.

This is a case where the line was lowered.

For this patient, the line was lowered using the ‘two-line method’ (Dujul-ttagi).

Looking at the pre-operative photo with eyes closed, you can see the existing scar located high up.

When lowering the line, the ideal method is usually to create a new line below it and then excise the tissue between the new line and the old scar.

However, in many cases, this method does not allow the line to be lowered significantly.

For instance, if the line is extremely high like in this patient, excising all of this (the yellow area) could lead to side effects.

The eyes might not close properly, the eyelashes and mucous membrane might be severely upturned, or the line might simply fail to lower.

Therefore, this patient underwent surgery using the ‘two-line method,’ which lowers the line while preserving the existing scar.

As a result, the original scar cannot be completely removed.

Structurally, the old scar will inevitably be visible above the new line.

However, when the eyes are open, the scar is not very noticeable.

It is only faintly visible when the eyes are closed.

People often ask about this scar; if the original scarring was severe, the upper scar might appear slightly raised.

If the scarring wasn’t severe and the skin is thin, it tends to flatten out,

but if the skin is thick and the previous scar was hard, some protrusion may be visible.

Cases where the line is too high, creating a ‘sausage’ look

In this patient’s case, the double eyelid line was too high, resulting in a ‘sausage’ appearance.

These are photos from before the surgery and about 6 months after.

The line was very high even at the inner corners, creating a heavy look,

so we lowered the line as much as possible to create a more natural appearance.

When the eyes are closed, you can see the existing scar is depressed in the upper area.

In particular, the scar at the inner corner of the right eye (on the photo) is very difficult to remove.

We try to flatten the internal tissue as much as possible, but the scar does not disappear entirely.

If you look closely, the parts that protruded before surgery may still show some traces afterward,

but the scar is not visible when the eyes are open.

Case 1: High line combined with ptosis, requiring ptosis correction

This patient had a high line, asymmetry, and a slightly sleepy appearance.

Because there was mild ptosis, we performed ptosis correction to make the eyes look clearer while lowering the line.

When the double eyelid is thick, ptosis correction alone can have the effect of slightly lowering the line.

This utilizes the principle that if you open your eyes weakly, the eyelid looks thicker, but if you use more strength to open them wider, the fold appears smaller.

If a patient wants to lower it even further than that, we can use the two-line method or scar excision as mentioned before.

This is the appearance 6 months after the eyelid line lowering surgery.

Case 2: High line combined with ptosis, requiring ptosis correction

This patient had a very high line and ptosis, so ptosis correction was performed simultaneously.

This patient also underwent the ‘two-line method’ surgery.

In the pre-operative photo, there was a depressed scar at the inner corner of the eye; this will never completely disappear.

However, we make every effort to flatten it as much as possible to improve its appearance.

But again, the scar itself does not vanish.

Cases with mucosal exposure (eversion) and multiple folds

This patient presented with multiple complex issues,

including asymmetrical eye-opening, multiple folds on the upper eyelid,

and significant mucosal exposure (upturned lid).

The line was lowered like this. If you look closely at the pre-operative state, there is a depressed scar at the inner corner.

As mentioned in other cases, depressed scars at the inner corner do not disappear.

It is important to know that these do not necessarily improve with time, so you must be careful not to receive a surgery that results in such deep indentations in the first place.

These are extremely difficult to correct.
